FACT - Families Achieving Change Together
Family Support & Young People
FACT operates on an early intervention model, working systemically to support the families meeting positive and achievable changes. The Family Support Workers on the project will work with young people aged 11-18 years and their families on a one to one basis as well as group work.

Men's Shed
Older Adults
Men's Shed
Our Men's Shed is where older men can "rub shoulder to shoulder" through activities including woodwork, craft making and information sessions. Our members share tools and resources they need to work on projects of their own choosing at their own pace and in a safe, friendly and inclusive venue.

Monday Club
Older Adults
This weekly club brings together older people through a range of activities and events including welfare rights, physical activity, health promotion activities, arts/crafts cooking demonstrations, bus trips and informational sessions. It takes a holistic approach to health that aims to promote social inclusion, reduce isolation and provide practical advice and support to the most vulnerable in the community.

Social Cafes
Older Adults
Our small social cafes are an ideal way to introduce new participants to new activities in a safe and secure environment and with the support of our volunteers. Participants come together for a cuppa and chat and identify activities together that they would like to undertake, for example, arts and crafts, light exercises, meditation, reminiscence, information and advice.

Stepping On - Falls Prevention Programme
Older Adults
The Stepping On programme offers exercise classes for those aged 65+ who have had a recent fall or at risk of falls. The 9 week programme programme is delivered in partnership with the Public Health Agency, Physiotherapy Team, Occupational Therapy Team, WHSCT Health Improvement Team, Western Home Environment Assessment Team and Healthy Living Centres.

Urban Villages - Primary Schools Resilience
Family Support & Young People
There is good evidence about what works to build protective factors such as self-esteem, positive physical environment and reduce risk factors in the schools, in order to promote well-being and some evidence specifically on building resilience.
